CEO Mary Barra has announced a "Speak Up for Safety" internal programme, to remove perceived and real barriers to candid conversations between employees and their leaders.

General Motors (GM) has added an issue with ignition lock cylinders to its safety recall of 2.59 million cars worldwide for an ignition switch problem. A GM statement says that the cylinders can "allow removal of the ignition key while the engine is running, leading to a possible rollaway, crash and occupant or pedestrian injuries". GM is aware of several hundred complaints of keys coming out of ignitions and has found indication of one rollaway in a parking lot that resulted in a crash and one injury claim, but no fatalities.
